The Adult Foster (Family) Care Program provides support to individuals in the comfort of their own homes or with their families. Our team of experienced case managers and registered nurses work with your family to help you care for your loved one. Our staff members come from diverse backgrounds and speak multiple languages.

Program Eligibility

  • For individuals receiving care
  • Must live with a caregiver in a private home or apartment in Massachusetts
  • Must be 16 years or older
  • Require assistance with daily living activities, personal care, and behavioral supports
  • Must qualify for MassHealth (Standard or Common Health) or a version of Senior Care Options (SCO)
  • Must have a doctor’s order showing a medical, mental, and/or physical need

For caregivers

  • Must be at least 18 years old
  • Must live with the person in need of care
  • Cannot be a spouse, parent of a minor, or legal guardian of the person in need of care
  • Must pass a CORI (Criminal Offender Record Information) check
  • Must follow AFC regulations

Caregivers may or may not be family members and will provide daily support to the individual.
